We understand that choosing the proper service can be a challenge. Here are definitions to assist in finding and selecting the right service to meet your needs.
Nursing Facility/Care Centers
Nursing home residents generally have physical or cognitive impairments that require daily assistance and keep them from living on their own. Skilled medical, nursing and rehabilitation help is provided. The facility offers 24 hour care to someone not able to live on their own.
Independent Living
This is a community designed for seniors who are able to function on their own but prefer the security and convenience of living with others. Recreational, educational and social activities are provided.
Skilled Nursing Facility
A facility or part of another facility that is primarily for providing skilled nursing care and related services.
Transitional Care Unit
This is often a necessary step between hospital and home for patients with complex and/or multiple problems. Transitional care helps patients make the move from medical dependence to self-care. It is for patients who no longer need hospital care.
Assisted Living
These facilities are designed for people who need daily assistance—bathing, dressing, laundry, housekeeping, medications, cooking—but wish to live as independently as possible for as long as they can.
Care Suites
Care Suites offer the comforts of home. The private apartment setting caters to clients who require services that cannot be met living in their own individual apartment. The Care Suites are staffed 24 hours each day with a high employee to client ratio. Each Care Suite offers clients quality comprehensive services to enhance their physical and psychosocial well being while remaining part of a larger senior community.
Memory Care/Enhanced Services
An assisted living option dedicated to the special needs of those with Alzheimer’s and other forms of dementia, including services for their families. Settings can vary. Memory Care can be provided at Nursing/Care Facilities, and Assisted Living Facilities. Activities, socializing, nutrition, and cares
Physical Therapy
Physical therapy (PT) works to improve and promote optimal physical mobility. Often treatment consists of exercise to improve strength, mobility training to improve transfer and walking skills, and balance training to minimize fall risks. Services can also include modalities to assist with healing and pain relief such as ultrasound, hot/cold packs and electrical stimulation.
Occupational Therapy
Occupational Therapy (OT) works to improve and promote optimal activities of daily living (ADLs). Often treatment consists of exercises to improve strength, ADL training to improve dressing, feeding and hygiene skills, and cognitive training to improve safety awareness. Services can also include obtaining, applying and training on positioning and adaptive equipment.
Speech Therapy
Speech Therapy (ST) works to improve and promote optimal swallowing and communication skills. Treatment consists of assessing for and making appropriate diet recommendations, exercises to increase oral motor and swallow strength, speech and communication training.
Wellness Program
A combination of programs intended to promote a healthy lifestyle which emphasizes physical, intellectual, social and emotional well-being through activities such as physical exercise and health education designed specifically for the older adult. Services are designed to develop and encourage improved healthy living habits, prevent and reduce the effects of chronic conditions and encourage physical fitness.
